Designed to be completed by the patient (either online, paper or via telephone) and centrally collated for interrogation of patient need and long-term risk ‘SAMS’ highlights:
- Individual symptoms (on a severity scale) that need attention
- Symptoms clusters i.e. low scoring symptoms that individually may not be of concern but collectively
indicate a ‘potential underlying clinical issue’. Each cluster is ‘tumour type specific’
- Symptoms trends to highlight the efficacy of a given intervention and predict problems longitudinally
- Other supportive care needs relating to vocational and psychosocial domains
Syndromes alert
Through the aggregation of INTACARE RM™ SAMS data, a Syndrome Alert is automatically populated that highlights an individual’s propensity to develop a long-term complication(s) or a co-morbidity e.g. Onco-Cardio, Metabolic , Musculoskeletal, Intra-pelvic and Neuro-cognitive Syndromes
Long-term risk reporting generator
An automatically generated report that includes:
- Highlights risk factor(s) contributing to an individual’s Syndrome Risk
- Identifies what the clinical implication of this risk is
- Recommends what objective outcomes measurements should be collected
- Provides a reference table of supporting evidence and relevant guidelines
Long-term risk management plan generator
An automatically generated plan recommending to clinicians what outcome measures to take and when, to enable the timely commissioning of services relevant to the individual’s risk profile
PAMS (Physical Assessment and Monitoring of Syndromes) Objective Outcomes Tool
A ‘plug & play’ application that provides a comprehensive set of tools for the collection and tracking of outcomes data recommended by the Risk Report.
Through the collection of objective data, PAMS facilitates:
- Measurement of patient’s physiological status and improvement or decline
- Indication of interventional need / interventional success
- Definition of co-morbidities
